The role

You will share the practice's behavioral health panel (currently 312 patients) with Salim Okonkwo, PMHNP-BC. You will be the second clinician on the panel, which we have been deliberate about — the panel has grown beyond what one clinician can sustainably hold, and we have been unwilling to enlarge it without enlarging the staff first.

Initial focus: psychiatric medication management, brief CBT/ACT, and (after orientation) MOUD prescribing for the panel patients with OUD. You will not be expected to carry the on-call BH pager during your first six months; we will pay your time on the pager rotation thereafter.

Who you are

  • NY-licensed PMHNP, board-certified ANCC PMHNP-BC.
  • DEA-registered. SAMHSA-recognized for buprenorphine prescribing (or willing to complete the MATE Act 8-hour training before start).
  • At least three years of post-licensure clinical experience in an outpatient setting that included some MOUD or addiction-medicine exposure.
  • Interested in spending the first months listening to how the practice writes its monographs, rather than translating an existing workflow into ours.
  • Comfortable being named as an author of the next revision of CCH-003 once you have something to add.

What you will inherit

  • An open and validated intake script.
  • An existing CCH-003 monograph with named contraindications.
  • A panel of patients who have been told, in writing, what the practice does and does not do.
  • A medical assistant team, a psychiatric pharmacist consultation arrangement (Wednesdays, by phone), a relationship with a CBT-p-trained therapist (Mira Ostrander, LCSW), and a working set of relationships with regional behavioral health resources (the McLean-aligned regional clinic; OMH-coordinated specialty mental-health programs; NYSPI).
