The list below is the entire clinical roster. There is no hidden tier of advisors, no fractional medical directors, no "supervising physicians" who do not actually see patients here. These five are the practice.
Maren Vanderlyn, MD
Family medicine focused on adult chronic disease and gender-affirming hormone therapy on the informed-consent model. Holds the medical director role since 2019.
Otis Thackeray, MD
Family medicine with a substance use disorder concentration. Initiates and maintains medications for opioid use disorder and alcohol use disorder. Carries the Tuesday and Thursday evening MOUD induction list.
Helena Rey, FNP-BC, IBCLC
Family nurse practitioner with a pediatric and lactation focus. Carries the pediatric panel as primary clinician; performs IUC and Nexplanon placement and removal as part of CCH-005.
Salim Okonkwo, PMHNP-BC
Psychiatric mental health nurse practitioner carrying the behavioral health panel. Co-treats with named community therapists; runs the brief CBT/ACT list.
Tessa Voorhies, DO
Family medicine with osteopathic manipulation training. Owns the office-procedure list (CCH-005) and the prenatal-coordination service.
Maritza Polanco

Practice administrator. MLIS, Pratt Institute, 2014. Holds the schedule, the formulary version control, the errata page, the front-desk script, and the calendar of when each monograph is due for review. Reads every revision before it is published.
Hiring discipline is something we do not always get right. We have hired two clinicians who did not stay; one because we asked them to do work that is not in any monograph here (we changed the monograph and improved the next hire's onboarding), and one because the practice was a poor fit for a clinician who wanted a high-volume schedule. Both are documented in the colophon's hiring history. The second of those errors is part of why the panel has been deliberately stable since 2023. We will not enlarge the panel without enlarging the staff first.
