Otis Thackeray spent the first nine years of his career in a federally qualified health center in Schenectady that adopted the Hub-and-Spoke MOUD model in 2014. He was the spoke physician for two years and then ran the hub-side intake from 2016 through 2019. He has done more than 800 buprenorphine inductions, both office and home, and is one of two clinicians at this practice who carries the MOUD panel.

His clinical interests are the medical care of adults with substance use disorders; the practical mechanics of getting buprenorphine into hands quickly and reliably; and the documentary discipline of explaining the boundaries of a practice to a patient who has been turned away from many other practices for less considered reasons. He is the named co-author of CCH-001 and the named MOUD co-author of CCH-003.

The practice's MOUD intake script — the questions Maritza asks at the front desk for any inquiry that mentions buprenorphine, methadone, or "OUD" — is his work. It is republished in the February 2026 bulletin on the post-X-waiver intake and is the most-cited internal document at this practice.

He is on call for adult medicine on a rotating basis (Mon, Tue, Wed, Fri) and runs the Tuesday and Thursday evening MOUD induction list. He covers the after-hours BH pager when Salim Okonkwo is unavailable and any post-discharge call related to addiction medicine.
